Our mission is to deliver the highest quality of care. Our goal is to return each patient to his or her most functional level of independence, with our ultimate goal of returning the patient to home, if appropriate, as quickly as possible.

Interstate 91 South: Exit 22S to Rt. 9. Exit to Rt. 66 West (Washington St.). Straight for 1.6 miles to 9th traffic light Boston Road on left.

North Exit 18 to Rt. 66 East: Follow Rt. 66 for approximately 5 miles, turn right at 2nd traffic light beyond Washington Plaza (Boston Road).

Route 9 North & South: Exit to Rt. 66 West (Washington St.). Straight for 1.6 miles to 9th traffic light Boston Road on left.
