Admissions
Perhaps your first contact with us will be before your admission when pre-arranging your short-term stay prior to elective surgery. Just call. Sue Stewart Director of Admissions, will make every effort to insure that you are well-informed and comfortable with the admission process. She will offer you a tour of the facility, an introduction to the Rehab staff who will be working with you here, and provide written information regarding what to bring and what to expect during your recovery at Glastonbury Health Care Center. She will verify your insurance coverage, let you know how long you might expect to be here, and answer any additional questions.
Patients hospitalized for illness, surgery or trauma will also be considered for short term rehab placement and families are asked to contact us, or ask your hospital discharge planner to call about bed availability as soon as hospital discharge is discussed.
Respite care is also available to provide relief for family caregivers for periods of time up to one month in duration.
Families of those considering long-term placement for a family member, or bed availability in our 'Lifesonnet' hospice program can expect a sensitive and comprehensive approach to an understandably difficult process. Your questions will be answered clearly...again we invite you to call for a tour, stop in for coffee, meet the team of health care professionals who have been providing this care so effectively for the past 18 years.
Rehabilitation
Our Rehabilitation Department, supervised by Physical Therapist, Diane Boxer, consists of Physical, Occupational and Speech Therapists and Physical and Occupational Therapy Assistants and Aides. This team of rehab professionals will be central figures in your recovery process, from the hour you arrive in our building, to the day you 'graduate' to your home. You will work with the same therapists each day, six days a week, to restore you to the point where you can return to the community.
Home care services will be put in place by our Director of Social Services, Dolores Matarese, before you leave to guarantee uninterrupted delivery of care...from the hospital, to Glastonbury Health Care Center, to your home.
